Kayleigh King (''Amber Dias-Ivanov'') is shown graduating from university with a degree in Business Administration. While her and her friends, Amy (''Lexi Anderson'') and Sophie (''Neiva Lo Cicero''), drink in a bar a couple blocks away, Kayleigh discusses finally making her parents proud. A man (''Barrett Highmore'') sits down near them and orders himself a drink and tells the bartender (''Gabe Gardner'') that his boss won't let up. The bartender tells him to continue at it since his boss isn't giving him any other choice. The man looks and stares at Amy, Kayleigh and Sophie as they leave. The three of them comment on the man staring at them as they leave. At Kayleigh's apartment, Sophie talks about her relationship with Mia and how well it is going. Amy and Kayleigh are in no rush to get into relationships, although Kayleigh does have a slight crush on Devon. The next day, the news discusses the disappearance of philanthropist, Anthony Da Rocha. Kayleigh messages Sophie about it as Sophie went to one of his charity events. Sophie is shocked and hopes he is okay. Kayleigh then hears the doorbell ring and goes to enter it but as soon as she opens the door, the screen goes to black.<br />

Kayleigh wakes up in a weird look room with odd architecture. At the end of the room is a large with "4" painted on it. A woman (''Bibiana Li Voti'') notices her and helps her up. The two of them gather at the door as others join them. Two of the men (''Barrett Highmore'' and ''Channing Renda'') begin screaming for whoever locked them up to release them. Others begin shouting as well. After a couple of minutes, a young man (''Valentino Carucci-Blake'') tells everybody that nobody is answering so it may be best to search for an exit themselves. When an older man (''Lorenzo Sashia'') says that he has no reason to go with anybody here as he doesn't trust them, the young man suggests that they introduce each-other. Another young woman (''Anrietta Sashia'') says that she refuses to give out her real name and says that everybody can call her "Venus". She would prefer people call her by a nickname as opposed to stalking her when they all escape. The film goes into how Adelio and Anthony came up with the experiment and what led Adelio to recruit his own daughter without her knowing. Titan is also revealed to be Adam Estes, an employee of King who was tasked with surveilling her during the last night, hence his appearance at the bar.<br /><br />
During the first experiment, Titan is seen in one of the chambers succumbing to the pressure. He talks to Citadel about wanting out of the experiment but Citadel tells him that he never had a choice. Titan tells Citadel that he will kill Kayleigh if Mr. King doesn't let him out of here. Citadel says that Mr. King would've kept her out of this if he cared enough to avoid a situation like this. This leads Titan to kill Ghost as seen in the first movie. Behind the scenes, Adelio King is furious as he would have preferred Kayleigh to die if it was at the hands of the experiment and not because of Adam's petty rage. This then leads to Titan's death at the hands of the trap, purposefully set by Adelio King to get back at him. It then shows Citadel's "death", which was set up before everything began so he could surveil the rest of the experiment.<br /><br />
The last 30 minutes is then "The Final Chapter", detailing the events after the third movie.

Bishop has just won the national chess championship, with the announcer saying that "Jana Carrington has done it again"! Halo is a journalist who is doing research on the "Puzzle Facility Incident". The survivors' names are listed as "Ellis Araujo", "Lauryn Rosa" and "Vanessa Vieira". "Halo" is now shown to be at a jeweler's store asking a woman about her daughter, Alexandra Rojas (Diamond). Cutthroat Agenda Season 4 (2018)

This leads Titan to kill Ghost as seen in the first movie.
