The title Empress of Lihnidos was created in 1675 following the declaration of the first Empire of Lihnidos. The first empress was Empress Theodora, the first child of King Konstantine IV of Thasos. Theodora reigned as Queen of Thasos for five years before declaring the empire in 1675. The declaration followed a three year war against the northern Kingdom of Ithaca, which ended with King Oceanus surrendering. Theodora allowed Oceanus to retain the title of King of Ithaca as a vassal under her following her declaration of the Lihnidosi empire. During Theodora's reign, after initially carrying over succession law from the Kingdom of Thasos, she instituted new law that only allowed females to inherit titles in the empire. This resulted in all titles eventually passing to females. Later Laskaris rulers conquered much what what is considered today to be modern day Lihnidos.

Vasiliou dynasty
On 15 July 1752 rogue members of the imperial guard assassinated Empress Maria II, her husband, and several members of her family. The first three in the line of succession were among those killed. The fourth, Maria II's second daughter, Stacia, fled to Tarsas with her husband, four children, and uncle. With the heir to the empire missing, the throne sat vacant during a tumultuous time. On 27 August 1675 Konstantina IV of Ithaca declared that, absent a clear heir, she would be claiming the throne. Loyal vassals in the Kingdom of Thasos rejected Konstantina as the new empress, and in late September a succession crisis began when forces loyal to House Laskaris attacked Ithacan soldiers in a small town on the border of the kingdoms. The small incident resulted in a mass mobilization of Ithacan forces into Thasos. By mid October, the majority of small cells of Laskaris supporting forces were wiped out and Konstantina IV became Empress Konstantina I of Lihnidos. The succession crisis did not end until late May of 1755, however, as in late October 1752, Tarsas forces attempted an invasion of Lihnidos to install Maria II's daughter Stacia on the throne. The war resulted in Tarsas being pushed out of Lihnidos after Lihnidosi and Latin forces joined to combat the threat. Tarsan forces eventually surrendered, recognizing Konstantina as the empress and withdrawing their forces.
It is widely believed that Konstantina had a role to play in the assassinations of Maria II and her family members, however no proof has ever arose to prove the suspicions.
Konstantina, having only male children, changed succession law to gender-neutral primogeniture. Her son who later became emperor, Isidoros, again changed the succession law from primogeniture to a method where the monarch chooses their desired heir from among their legitimate children. This resulted in Isidoros's eldest child, Stathis, being passed over in favor of Isidoros's next eldest child, Kassia. Kassia later signed a proclamation that ensured her children, as well as all future descendants of future female monarchs or title holders in the imperial family, kept the Vasiliou name. This method continued until 1834 when Selene I became infirm due to failing health. Having created a council of nobility following the adoption of a constitution in 1803, the council ruled as a collective regency council. Selene's chosen heir died that same year leaving her the responsibility of selecting a new heir. She was not of sound mind at the time and did not select a replacement. This led the council to give themselves additional powers and effectively changed the succession laws without consultation of the sitting monarch. Having implemented a form of elective monarchy in which the council selects a child of the monarch to rule after the monarch's death, the council chose one of Selene's granddaughters, Konstantina. Konstantina took the throne on 2 November 1834 after the passing of her mother. Konstantina chose to keep the change of succession law after implementing minor changes. This continues to be the method that heirs to the throne are chosen.
