General elections were held in Los Angeles on 7 May 2023 to elect the members of the Presidium. At stake were at 602 seats in the Presidium, as well as a variable number of overhang and leveling seats determined thereafter. The Presidium uses a modified form of ixed-member proportional representation which includes the halving of the Presidium's membership. With overhang seats there were 1,206 members, though only 606 would be allowed to take their seats.

The Conservative Alliance, made up of seven regionally-based center-right parties, and led by Luca DiFelice, won the lowest percentage of the vote for a winning party at 29.82%, edging out the Communist Party's 24.67% under incumbent general secretary Charles Maggle. Swings against the AdE, AdPE, and several other mainstream parties in favor of the right-wing populist Democratic Party and several secessionst parties. A total of fourteen parties or alliances (twenty eight parties if the Alliance and AdPE are broken up) won seats in the Presidium. In the final 606 member Presidium, a majority is 304 and the Alliance won 184 seats, the Communist Party won 152 seats, the AdE 59, Popular Union won 44, the Democrats 35, Liberty 29, AdPE 17, PdEN 15, the FoF/FoV 14, Birthday 14, United Native 13, Citizen's Forum 12, Rally for Our Language 10, and the New Ketha Communist Party 8.

The first ever peacetime grand coalition was formed in Los Angeles between the Communist Party and the Alliance, with the Citizen's Forum to mediate with its leader, Ricardo Johannes being selected as general secretary and the new government took place on 13 June 2023. The agreement came after the Alliance had decided that a traditional center-right coalition would be untenable, even with the support of the increasingly right-wing Democratic Party. With the Alliance refusing to enter into a coalition with any secessionist parties, a grand coalition with the Communists was formed instead.
