The Prime Minister of the Pacitalian Republic is the head of government of Pacitalia.

Typically the leader of the political party with the largest number of seats in the Constazione Repubblicana (lower house), the prime minister appoints a "cabinet" of ministers, the Executive Council, to serve in their government, and chairs the council. The prime minister is responsible for setting the government's policy agenda and overseeing the work of their cabinet.

After an election, or upon a vacancy, the Archonate of the Pacitalian Republic, Pacitalia's head of state, nominates a formateur, who they believe can command the confidence of a majority of members of the Constazione. The formateur achieves this in one of three ways: by their party holding a majority of seats in the Constazione, by reaching a formal coalition agreement with other parties, or through a memorandum of understanding with parties who are prepared to support the governing party on key votes. The last option is the least stable, and riskiest, for a government.

The formateur's candidacy for Prime Minister is debated by parliament; the candidate is then subject to a vote of investiture in both houses. If successful, the candidate is immediately, and constitutionally, the Prime Minister, at which point they can present their proposed cabinet for investiture. Lawmakers approve the cabinet as a whole, rather than confirming each minister individually.

Prime ministers can "shuffle" their cabinet mid-term by dismissing some, or all, of their ministers, and having the Constazione vote on a proposed new cabinet, but if the measure fails, the government is deemed to have lost confidence, and the prime minister is left no choice but to tender their resignation to the Archonate.

As the head of government, the prime minister's remit is overwhelmingly domestic affairs, in contrast to the Archonate, who is the primary government official for foreign matters. The Minister of International Relations, or their equivalent, is a key portfolio in the prime minister's cabinet, leads the similarly-named Directorate, and coordinates the government's efforts in multilateral affairs and international diplomacy, but remains junior to the Archonate.

A prime minister's term lasts up to three years, until the next required parliamentary election, but prime ministers are not subject to term limits, given the need for them to repeatedly test the confidence of parliament to remain in office.

The current prime minister is Italo Pavoni, who was appointed as formateur by Archonate Abeo Bamidele on December 6, 2024, and subsequently took office three days later, inheriting his predecessor Damián Moya's cabinet. He is a member of the Pacitalian Social Democratic Congress.
