The CAD's candidate Giuseppe Rognoni lost the 2008 election to Dario Lorenzin's Alliance for the Centre. However in 2010 the AC collapsed when the right-wing faction of the Socialist Party broke off to form the PSP, with the PSP and Forza Marirana leaving the AC and joining CAD. This gave CAD a majority in the legislature resulting in the coalition to force president Lorenzin into cohabitation.

In the 2013 presidential election the CAD candidate, Giorgio Macciocchi, won with the CAD retaining its parliamentary majority. In 2017 22 Councillors and 5 Senators split from the CAD to form the Union of Centrists and Democrats, resulting in the CAD to lose its majority in the House of Councillors.

During the 2018 presidential election the CAD's candidate Rosa Michelozzi came third, marking it the first time since 1936 that a member of the Anti-Revolutionary party did not advance to the second round of an election. The CAD in the 2018 legislative election saw a devastating defeat losing the majority of their seats and coming third to Avanti Marirana! and Popular Force 2018.
