The film begins with the Sikkema family each individually waking up, Andrus and his wife Laaske leave to get groceries and witness an increased military presence encouraging the public to take social distancing measures. We find out via the internet that a highly infectious new disease similar to {{wpl|Rabies}} has appeared on the island following a fire at a medical facility earlier that week. The government is heard urging the public to stay calm. The two enter the shop shortly before emergency sirens go off across the island and an automated voice urging people to seek shelter immediately and avoid the 'infected' at all costs. Andrus looks outside to see a group of about 10 'infected' run into the car park and attack dozens of people there and overrunning the military checkpoint.
20 Days after the release of a deadly virus in a reservoir in Southern [[Gaullica]] most of the population of Euclea has been annihilated by humanoid 'infected' which possess a heightened sense of strength and speed.


Laaske calls their teenage daughter Sytj and tells her to hide in the attic with her brother Goska. Sytj gets Goska into the attic and witnesses a group of infected attack a man in the street in front of the house. Back in the shop Andrus and Laaske make plans with the shop owner Hans and a young couple, Grace and Adam to make it to Noardlikein where they hear over the news that evacuations are taking place. The five leave the shop through the back entrance and discover a young woman being attacked by an infected soldier. Hans manages to kill the soldier but not before the Young Woman turns and attacks and kills him. Adam, Laaske and Andrus flee along with Grace. The group make it to a military checkpoint where a Soldier shoots Adam after mistaking him for an infected.
The Kuiper Family - wife Uwkje, husband Its and children Alemtsje and Atze scavenge for supplies in an abandoned village near Rikertsted. The family had been living in  an abandoned School although are forced to leave after running out of supplies. Its tells the family as they are leaving that the countryside is full of infected who had tried fleeing to rural areas in the early days of the outbreak. After following a set of train tracks they meet another group of survivors, Ellen and Stephan. Ellen tells them that there is a supposed safe haven in Fawkhamshire and that she will guide them there if they can join their group. The family agree although Its is suspicious of the two. The group kill a sheep to eat although Alemtsje complains about the food. The next day they scavenge for supplies and Its enters a house to find edible food for the family. He is killed after another survivor mistakes him for an infected and kills him. The other survivor is then subsequently killed by an infected after a fight breaks out between him and Uwkje and the group flees.


The film cuts back to Sytj and Goska who get their neighbours into their loft through the back garden. The Neighbours inform the two that the island is under quarantine and the bridges connecting the island to the rest of the city have been destroyed. The group hear the infected break into the house and escape onto the roof. Sytj calls Laaske who tells them that they are on their way and to stay away from the neighbours.
Stephen and Ellen are revealed to be married and had left Yndyk on the seventh day of the outbreak as martial law is declared by the Alish government. They then claim their daughter was shot and killed in the chaos as they ran from their car on the motorway which was full of cars also leaving the city. Alemtsje and Stephen develop a close bond as they catch fish from a river to eat.  


Andrus, Laaske and Grace find an electric car and begin driving to the house passing by several scenes of destruction as the island has fallen into chaos. The group get to the house to find that the neighbours had been infected and Syjt and Goska having disappeared. Laaske manages to find the two hiding in the garage and gets them to the car with Grace and Andrus who had attempted to subdue an infected with a Knife but ended up injuring himself. The group make it to the car and drive away just as a large group of infected arrive. The car runs into a large traffic jam near Noardlikein with many people abandoning their cars and fleeing towards the boat. The group leave the car just as more air raid sirens sound causing mass panic as the infected approach.
The next day they cross a river and find a group of infected next to an abandoned train. Atze's panicked screams alert the trains infected occupants to their location and a large group of infected begin to chase them across the bridge. Atze disappears during the chaos and the group find her body on the riverbank. Uwkje becomes distraught after the death of her husband and daughter and attempts suicide but is stopped by Ellen who admits that she and Stephan aren't married but she had to kill her husband as they left Yndyk.


Aandrus who had been injured earlier stops running due to his injuries and has to be carried by Laaske, Goska and Grace. The group get onto the boat just as it leave the port leaving dozens of survivors behind to be infected. Aandrus is taken to the medical room on the boat where it is discovered an infected person had gotten onto the boat causing panic. Andrus calls Laaske telling her to get Goska, Syjt and Grace to a lifeboat as the infection begins spreading throughout the ship. Mass panic on the boat causes a bottleneck on the stairs to the lifeboat. Aandrus escapes the medical room through a vent and gets onto the deck. Grace is infected in the stairwell as the rest of the group escape onto the boat deck. On the deck they realise the captain is turning back towards a group of rocks attempting to sink the ship to avoid the infection spreading to the mainland. The boat strikes the rocks causing it to begin capsizing quickly as lifeboats are being launched. Goska is thrown from the deck into the river by the boat tipping. Aandrus, Laaske and Sytj follow suit and begin swimming to the shore. The four are rescued along with other survivors by the military and put in quarantine.
They make it to the supposed safe haven which turns out to be a village next to a lake. Ellen runs towards a house on the lake shore and finds the bodies of three people. Ellen is distraught and collapses into tears. She admits there was no safe haven and that she had been trying to rescue her daughter who had been staying with her Grandparents. An enraged Uwkje attacks and kills Ellen and severely injures Stephan with a Kitchen knife. Stephan bleeds out from his injuries. That night Uwkje allows herself to be infected by drinking from contaminated drinking water in the tap. Alemtsje flees the house and finds a group of survivors near the Estmerish border who take him in.
 
Two months following the events of the film we find out the virus has spread to the mainland with the government urging people to stay calm and saying it's all under control.
