The Prime Minister of Cacerta is the head of the Government of Cacerta. The office of Prime Minister was created following the Unification Wars and the creation of a single Cacertian State. The position of Prime Minister was first held by Madalena Vannuci, a prominent military commander of future King Victor I. She held the position for approximately four years before Victor’s death and decided to retire from politics following Victor’s daughter’s accession to the throne. The longest serving Prime Minister was Filomena Marcello who obtained the position in January 1939 and served through the Great War until December 1950.

Amika Carpio is often considered responsible for beginning social reforms that laid down the early foundation of Cacerta's modern-day welfare state. Regarded historically as the Peoples' Prime Minister, Amika worked closely with Cacerta's powerful noble houses to pass workplace safety legislation, health insurance programs, and old age pensions, as well as encouraging cooperatives and social ownership.

Cristina Vernati set important constitutional precedents after the divided Parliament of the 1900 election. She informed Elliana that she could form a government despite the Ultranationalists having a majority in the House Assembly, and was appointed Prime Minister, establishing the Common Assembly as the responsible house. Her introduction of a land tax in the 1903 budget, passed over the House Assembly's opposition, led to legislation that weakened its ability to block legislation passed by the Common Assembly.

Elena Viareggio introduced the proporzionalità ("proportionality" — also known as lottizzazione, "dividing into lots") system into the Cacertian public sphere. The system allowed the opposition parties and Houses to share roles in the administration of public policy and state-owned enterprises. It was introduced as a reconciliation measure following the political tensions of 1900 and 1905.

Fabiano Picerno led Cacerta through the Divide War with Syara. At the war's conclusion, he publicly accepted responsibility for Cacerta's defeat and retired at the end of his term. He implemented a national income tax for the first time, to fund the war effort. The tax was not repealed and made permanent by his successors, who used it to fund domestic improvements.

Michelle Luzio led Cacerta through the regional depression which struck in the 1930s. He responded to the depression with a program of deficit spending and reduced interest rates to stimulate the economy, and extensive public works to fight unemployment. He contemplated breaking the two-term tradition and running for a third term to continue leading Cacerta out of the economic crisis, but refused to due to the global situation.

Cesare Ferrera continued his predecessor's economic stimulus policies, and began a program of modernisation and re-armament of the Cacertian military in response to rising regional tensions. He stepped down as Prime Minister after the unclear result of the 1938 election, remaining as caretaker until Filomena Marcello was able to form a government.

Although the Empire was not officially disbanded until 1940, it is generally accepted that the new Kingdom era begian in 1938 following the death of Empress Elliana I.

Filomena served as the final Prime Minister of the Cacertian Empire and oversaw its devolution over the course of her first year in office. She went on to lead the wartime National Ministry that was formed following Cacerta's declaration of war against Quenmin (and later Acrea). The 1943 election was suspended by order of Queen Rosalia as a result of the ongoing conflict and Filomena remained Prime Minister. After the war, Filomena led the nation's reconstruction effort and was re-elected in 1945 before retiring from politics at the end of her term in December 1950. She is the longest serving Prime Minister in Cacertian history.

Modesto Fratini presided over a critical expansion and transformation of the Cacertian welfare state, from a liberal, insurance-based model to a universal, social democratic one. He expanded public education, set up a single-payer universal health care system, and increased and broadened welfare payments. He nationalised many companies that had previously been owned by particular Houses, making them state-owned enterprises.

Verdiana Castiglioni built on her predecessor's policies, and is remembered for her "National Development Program" (Programma di sviluppo nazionale). The plan entailed subsidies to new industries to broaden Cacertian exports, and an ambitious infrastructure program to "rebalance" regional development through a new network of railways throughout the country and transferring more economic functions to smaller cities.

Masina Calvenzano brought Cacerta into the Common Sphere. In the domestic sphere, she began building high-speed railways, a policy which benefited from closer economic ties with Kirisaki and availability of experts through the Common Sphere. During her term, the new social movements of the 1960s began to reverberate in Cacerta, with the emergence of a leftist student movement and an ecology movement.

Asella Cottone began the implementation of the Hermes Programme in Cacerta, which became a valuable tool for indicative planning, and deepened economic ties to fellow Common Sphere member states. Piero Naldi led Cacerta's first minority government, with the support of the Labor Party. He set up the fondi salariali ("wage-earner funds") — trade union-administered funds financed by the issuing of new stock, which would gradually transfer majority control of firms to their workers — and the scala mobile ("escalator"), a mechanism by which wages, pensions, and benefit payments were indexed to inflation.

Cristostomo's premiership came after nearly 27 years of Unitary Party dominance of the Parliament. His nine-year career in office under the reign of King Agostino I began the process of opening up the nation after a period of long-term isolationism under King Doriano I. It was believed that his implementation of progressive foreign policies would guarantee a third term in office (unprecedented in previous history). However, he was defeated by Henrietta Ianelli during the 2006 General Election following the unexpected death of the King. He went on to serve as Henrietta's Deputy Prime Minister during her time in office between 2006 and 2016. After the 2016 General Election, Vincencio retired from politics.

Henrietta is the first House Viareggio Prime Minister since Cesare Ferrera and also the first since the House's political shift from Ultranationalism to Unitarianism. Despite being a Unitarian, she supported Queen Anelyn's initiative to work on Cacerta's lackluster foreign relations programs and was a key individual in exploring closer relations with the nearby nations of Gylias and Kirisaki. She has been criticized for her handling of the Nalayan Civil War which many believe partially resulted in the falling out of the Unitary-Labor Coalition that defined her time in office. She is currently serving in the position of Deputy Prime Minister under Valeriana Khushrenada.

Valeriana, a member of House Khushrenada, gained significant influence in the Ultranationalist Party following the December Schism that saw the creation of the more moderate Unionist movement. Working closely with Ultranationalist leader Iralia Marik, Valeriana became a leading figure in the neo-Ultranationalist movement which saw the revitalization of interest in foreign policy as a result of the Nalayan Civil War. She leads the nation's first Grand Coalition government and has made significant efforts in improving relations with Shalum and the fellow nations of the Common Sphere.
